US investigators have disclosed that the pilot of a De Havilland Canada DHC-6 Twin Otter, which crashed during a skydiving flight in Tennessee, had been attempting to return to the airport after apparent loss of engine power on take-off.
Air India flight AI171, which crashed in Ahmedabad on June 12, experienced a dual engine shutdown seconds after take-off, according to a preliminary report released by Aircraft Accident Investigation Bureau (AAIB) on the fatal crash involving Air India’s Boeing 787-8 Dreamliner.
The Aircraft Accident Investigation Bureau (AAIB) has reconstructed the final 90 seconds of Air India flight AI171, confirming that both engines shut down seconds after the Boeing 787-8 lifted off from Ahmedabad on 12 June 2025.
A US based aviation attorney told FinancialExpress.com that a TCMA failure could be the cause behind the Boeing 787 crash. The London-bound Air India flight AI 171 crashed in Ahmedabad moments after take-off on June 12.
"Aircraft systems are designed to alert pilots to any faults prior to take-off. Given these safeguards, any in-flight failure is most likely due to a sudden event -- such as a bird strike or a technical malfunction," says the Air India pilot, speaking on condition of anonymity.
